Please complete all information pertaining to your credentials, presentation description and presentation objectives.. General session presentations will be a total of 45 minutes with 10 minutes of Q & A.

Once MAPA receives the CME submission, it will be reviewed by the CME committee for comment and approval. The MAPA CME committee will then contact the presenter regarding a speaker agreement and honorarium.

MAPA offers one ofthe following options for Honorarium:

• $100 Honorarium
• 1 Night hotel stay
• Conference Registration for Day of Presentation (Includes CME credit)
• 1 year MAPA membership
