New QI cohort to support clinicians

The Minnesota Department of Health (MDH) would like to support you in improving the screening and referral processes you use in your care of a pregnant or postpartum mother and/or of a new baby. This could be in a clinical setting, family home visiting, public health, or any place you are seeing new or pregnant moms.
